Position Highlights:

Community Health Worker
Location: Swedish Hospital - Chicago, IL
Full Time, 40 hours/week
Hours: 8:00am-4:30pm
Monday - Friday

What you will need:

High school diploma, GED or equivalent.

Community Health Worker, Health Coach, Patient Advocate Certification and/or equivalent preferred.

Minimum of 2 years community based experience.

Basic computer skills required. Experience with EPIC EMR preferred.

Fluency in Russian, Spanish, Urdu, Hindi, or Arabic a plus.

General Summary:

Passionate and dedicated public health worker with a deep commitment to improving the well-being of individuals within the community. A trusted liaison and advocate, adept at establishing strong connections with patients and their families while providing culturally appropriate support and encouragement. Skilled in conducting assessments, providing health education, and offering culturally sensitive support. Collaborates seamlessly with healthcare professionals, community organizations, and local leaders to address health disparities and enhance overall community health. Proven ability to navigate complex healthcare systems, build relationships, and empower individuals to make informed decisions about their health. Committed to promoting preventive care and fostering a healthier and more resilient community.

Responsibilities and Essential Functions
Assess and address patient's needs in the area of housing, food insecurity, transportation or other non-medical areas that influence a patient's health outcomes (Social Determinants of Health). (20%)
In partnership with patient care teams, provides support for patients in gaining access to medical services, screenings, prescription medication and durable medical equipment. (20%)
Assist patients with getting access to social services and other community-based resources. (20%)
Follows up with patients via phone calls, home visits and visits to other settings where patients can meet. (20%)
Maintain accurate documentation of patient contacts and document patient's needs, action plan and outcomes in the medical record system. (10%)
Represents the organization at various community events. (5%)
Participate in continuing education, trainings and CHW organizations. (5%)
