Position Highlights:

Manager - Community Outreach
Location: Swedish Hospital - Chicago, IL
Full Time, 40 hours/week
Hours: 8:00am-4:30pm
Monday - Friday

What you will need:
Bachelor's Degree or 5+ Years' Experience required
Preferred fluency in Spanish, Arabic, Korean, Vietnamese or another language.
Strong passion, leadership and interpersonal skills necessary to effectively work with multi-cultural, diverse populations.

General Summary:

The Manager, Community Outreach is responsible for strategic and operational leadership of Swedish Hospital's role in the Chicago Department of Public Health (CDPH)'s Healthy Chicago Equity Zones (HCEZ). The Manager's responsibilities include coordinating activities and programs in collaboration with community partners that support the requirements of the grant, overseeing and supporting assigned team members, and representing the organization in a variety of public settings to increase awareness and develop prospective additional community partnerships.

Responsibilities and Functions
Establish a Regional Healthy Chicago Equity Zone for the North Central Zone in collaboration with Community Leads; execute and manage subcontracts with Community Leads.
Develop and implement a strategic plan for Swedish's HCEZ work that collaborates with and supports other hospital and Community CARE initiatives
Manage budgets, ensure timely submission of expense reports to the City of Chicago and reimbursement to Community Leads, and provide fiscal and administrative support for Community Leads.
Build and maintain neighborhood profiles, engagement plans and communication materials with a focus on identifying specific populations requiring attention
Conduct outreach and engagement efforts, in collaboration with other area stakeholders and organizations.
Participate in CDPH HCEZ meetings and programs.
Prepare and submit required program reports that summarize key performance indicators and activities.
Have regular touch points with Community Leads, conduct site visits and audits, monitor performance against deliverables, ensure development of action plan to meet objectives, and assist with evaluation to strengthen the overall program.
Collaborate with other Community CARE programs.
Supervise Community Health Specialists and Communications Specialist, Community Outreach.
Other projects and duties as assigned.
