The Department of Pharmacy Practice at the University of Illinois at Chicago College of Pharmacy, is looking for a Clinical Faculty (Open Rank) / Clinical Pharmacist to join our Antithrombosis team. The ideal candidate will be responsible for providing pharmaceutical care to patients in the area of antithrombosis management at the University of Illinois Hospital & Clinics. The clinic sees patients with major disease states requiring short or long-term anticoagulation therapy with traditional and direct oral anticoagulants including patients s/p major orthopedic surgery, venous thrombosis, ischemic stroke, peripheral vascular disease, atrial fibrillation, various hypercoagulable states. The clinic also sees high risk pregnant patients and pediatric patients in need of anticoagulant therapy. Consults for peri-procedure bridging therapy are also handled. The hospital is a 450-bed academic institution located approximately 2 miles west of downtown Chicago. with 21 on-site outpatient clinics and 11 FQHCs. The hospital and clinics serve an urban patient population.

The College of Pharmacy at UIC, with campuses in Chicago and Rockford, is the #1 college of pharmacy in the state of Illinois and #15 in total research grant funding according to the American Association of Colleges of Pharmacy (AACP). The clinical pharmacists in ATC work under the Antithrombosis Therapy Management Protocol for all items including drug initiation and selection, ordering and monitoring labs, dose adjustments, ordering refills, discontinuation of therapy, and patient education. Each clinic encounter is billed using facility fee charges. The candidate will also be responsible for nurse, physician, and pharmacy staff development; coordinating and conducting quality initiative projects and developing collaborative treatment and research protocols. The new hire will be assigned to ATC 4 days per week with 1 office day per week. The hours will be Monday - Friday during the day.

The academic responsibilities of the clinical faculty member (Clinical Assistant/Associate/Full Professor) include the provision of didactic lectures and precepting entry level Doctor of Pharmacy students, PGY1 residents and PGY2 residents. Scholarly activities, including presentations and publications, are an expectation as well. Academic rank commensurate with experience.

Minimum Required Qualifications:

PharmD and PGY1 residency experience required.

Candidates must present evidence of, or eligibility for, Illinois licensure as a professional pharmacist at time of employment.

Highly preferred strong clinical knowledge base; analytical and problem solving skills; strong verbal and written communication skills; ability to consistently meet deadlines; demonstrate flexibility and ability to work independently and adapt to a changing work environment; excellent planning, time management and organizational skills.

Academic rank commensurate with experience.

Preferred Qualifications:

PGY2 Ambulatory Care residency or 2-3 years of equivalent clinical experience is preferred

Board certification is recommended
