Duties and Responsibilities

Primary care physicians boarded in Internal Medicine or Family Medicine needed to part of a House Calls program making home visits in Chicago and suburbs of Western Cook County. Full-time and part-time positions are available. The measure of success for this position is improved patient healthcare outcomes, leading to a lower admission and readmission for this patient population. This position will part of a team, with nursing and social work support, managing a panel of homebound patients.

MacNeal is a 374-licensed-bed teaching hospital in Berwyn with advanced medical, surgical and psychiatric services, acute rehabilitation, an inpatient skilled nursing facility and a 68-bed behavioral health program and community clinics.
