PD CLINICAL PHARMACIST

Job Description

Reporting to the Director of Pharmacy or designee, and in compliance with Federal and State regulations and established hospital and departmental policies and procedures, performs a variety of professional pharmaceutical functions to provide for the safe and effective preparation, storage and dispensing of drugs. Directs and monitors the work of pharmacy technicians.

II. REPRESENTATIVE FUNCTIONS:

Accurately interprets physicians' orders and enters drug information into the computer. Reviews, maintains, and screens patient medication records and profile for allergies, drug interactions and therapeutic duplications when entering new medication orders.

Contacts physicians when necessary to clarify and/or correct an incomplete or questionable order. Consults with physicians and nurses concerning drug availability, dosage, adverse drug reactions, drug incompatibilities and other drug information inquiries.

Supports and interprets hospital and pharmacy department philosophies, objectives, policies and procedures. Seeks the guidance and assistance from superior, supervisor, or coordinator when unable to interpret policies because of a lack of knowledge or experience or unable to enforce them because of a lack of authority.

Demonstrates a working knowledge of current pharmaceutical literature and practice for all age groups of patients served by the hospital, and makes sound clinical decisions based on this knowledge.

Processes drug orders in a manner to assure departmental quality standards of timeliness and accuracy are met.

Accurately checks all drugs prepared by pharmacy technicians before the drugs leave the pharmacy.

Provides drug information in response to practitioners' questions and patients' needs.

Participates in pharmacy staff meetings and council meetings.

Adheres to departmental and hospital safety standards in order to maintain a safe patient care and working environment. Attends all mandatory safety in-service education programs.

Assumes responsibility for own professional growth and keeps abreast of developments in the pharmaceutical field. Provides documentation in writing to support the fulfillment of continuing education requirements for re-licensure yearly at evaluation time.

Adheres to the hospital's guest relations policy. Interacts positively with others and works cooperatively with both pharmacy and hospital staff. Answers the telephone in a friendly, courteous manner.

III. SPECIAL SKILLS AND ABILITIES REQUIRED:

The analytical skills required to interpret physicians' orders and to review patient's profile to avoid overdosage or toxicity.

The communication skills necessary to deliver and obtain information in a clear, concise manner.

The interpersonal skills necessary to interface with physicians, nurses, and other customers in meeting patients' pharmaceutical needs, and to direct the activities of pharmacy technicians.

The organizational skills necessary for day-to-day coordination of the pharmacy service.

IV. KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

B.S. in Pharmacy or Pharm D degree.

Current license to practice pharmacy in the State of Illinois.

One or more years of experience in an applicable pharmacy practice site.

BLS and ACLS certification within 3 months of hire required.
