Maternal Child Health Advocate/RN

Southern Seven Health Department is seeking applications for a regular, full time Maternal Child Health Advocate/RN position that is based at the Massac County Public Health Clinic in Metropolis, IllinoisThis position does come with a benefit package, including vacation, personal, and sick time accrual, holidays, health and life insurance, and an employer sponsored cafeteria plan. This position performs core case management activities of assessment, planning, referral, advocacy, and follow-up.

Qualifications: The applicant must have a Master's or a Bachelor's Degree in social work, behavioral science, social science, or a health related field. Preferences will be given to candidates with case management experience.

OR

A valid Illinois RN license; preferences will be given to an applicant with previous experience in case management activities.

Pay Range: $21.32-$22.39

Job Duties:

  • Provides case management services to assigned pregnant women, infants and at risk children.
  • Completes a comprehensive needs assessment and individualized care plan on all families enrolled within a required time frame.
  • Performs home visits on all at risk clients and provides appropriate referrals as needed.
  • Works with the nursing staff to perform and document screenings on all women for post-partum mood disorders, developmental screenings, and Well Child/EPSDT exams, and refers to services as appropriate.
  • Provides keyboard data entry of client data and medical care coordination into online data base program.
  • Maintains client files including filing duties.
  • Follows all policies and procedures related to confidentiality and HIPAA regulations.
  • Performs other duties as requested or assigned to meet the requirements of the agency.
