Position Highlights:
Position: Phlebotomist - per diem Tier II
Location: Glenbrook Hospital
Full Time/Part Time: Resource, per diem
Hours: Monday-Friday, days-5am-9am, e/o week-end and rotating holidays

A Brief Overview:
The Phlebotomist II perform a variety of routine and special phlebotomy procedures as authorized by the Manager/Director. Position is also responsible for ordering tests into the Laboratory and Hospital (LIS/HIS) computer systems and documenting Lab receipt of specimens. Position may also include processing of various Laboratory specimens.

Completes competency and work-related assignments within the established timeframe.
Completes performance self-evaluations and all related documentation in a thorough manner within the expected timeframe.
Follows all Infection Control and safety-related policies and procedures and uses appropriate PPE.
Is knowledgeable and compliant with Federal, State and local laws that apply to area of responsibility.
Follows the respective specimen collection etiquette policy when greeting patients in the OPL/PSC or inpatient units.
Provides services in a cordial manner to enable testing and enhance the patient experience.
Contacts physician for patient orders when they are not available in Epic or elsewhere.
Answers phone calls in accordance with the telephone etiquette policy. Answers questions and resolves issues to the extent possible.
Provides directions to patients on how to get to their desired locations.
Provides information on availability of results to patients and answers any other questions that the patient might have.
Upon request generates patient reports, and faxes or processes reports, as directed.
Takes verbal orders and completes all appropriate paperwork.
Follows up on verbal orders by completing the form, faxing to ordering physician and scanning returned form into EPF.
Researches esoteric testing requirements, when applicable.
Rotates between inpatient, outpatient and Patient Service Center (PSC) departments as assigned.
Collects patient specimens in accordance with test requirements and in a manner which is appropriate for the age and/or specific patient population served.
Verifies patient identification by requesting patient provide correct spelling of their full name and date of birth. For inpatients, ensures the proper wristband has been applied prior to scanning with Soft ID hand held device.
Minimizes patient blood draws for subsequent orders when appropriate.
Collects samples from adults, children and infants using venipuncture or finger/heel stick as appropriate.
For inpatients, employs SOFT ID to monitor workload, collect patient samples, print labels and/or reschedule testing.
Ensures all orders are reviewed for accuracy/completeness and all Call List and order comments are entered in the LIS.
Collects blood samples for specific tests ensuring volume is adequate and correct tubes are utilized. Labels all specimens appropriately with the correct label. Complies with blood minimization policies and procedures as appropriate.
Provides specific instructions to patients for patient collected specimens such as urine, stool, sputum, etc... Utilizes specific "Facts About..." as appropriate.
Collects non-blood specimens according to established procedure: nasopharyngeal, throat, nasal swabs and labels with correct specimen label.
Collects all STAT and timed test requests within 15 minutes of page or desired collection time when applicable. Communicates with nursing unit when delays are anticipated.
Participates in the collection of material for specialized testing (i.e. cystic fibrosis sweat test determinations)
Prepares patient specimens for testing including centrifuging, separating, storing, packaging and arranging for transport to alternate locations.
Packages specimens for transport to alternate Hospital location using established packaging/shipping policy and procedure and prepares tracking list. Reconciles transport list to specimens being sent.
Measures 24 hour urine specimens, records measurement and transfers aliquot to appropriately labeled tube. If applicable, performs pH measurement and records.
When indicated, splits and aliquots stool samples into appropriate containers.
Arrives patients in epic, releases or enters appropriate orders.
Collects and receives all specimens in SoftLab and enters venipuncture charges.
