Position Highlights:
Position: Social Worker
Department: Out- Patient Physical Rehab Services
Location: Rolling Meadows, IL
Hours: This position is 20 hours per week. Must be able to start at 8:00 on Wednesdays. Other part of schedule can be flexible! Can work 3 days or 4 days depending on preferred choice. No Weekends and No Holidays!
Shift: Days
Part time/Full Time: Part Time 20 hours per week
General Summary:
Provides Counseling Services
Reporting to the assigned Unit Manager, coordinates matrix relationships that involve collaboration with multiple healthcare team members.
Provides consultation and support to the team members on patient resource and psychosocial matters and concurrent review issues upon request.
Identifies community resources, coordinates financial and clinical aspects of health care
Maintains accountability for process improvements that reflect increased quality and decreased cost of care.
Will serve as patient advocate
Functions as an integral member of a collaborative and interdisciplinary team
What You will need:
Master's Degree from an accredited school of Social Work.
Licensure to practice social work in Illinois (LCSW)
Two years' post-graduate experience preferred; healthcare and/or mental health experience preferred
CPR from American Heart Association
