Position Summary

Under direction of the Associate Chief Clinical Operations Officer, Continuum of Care, the Continuum of Care Project Manager manages administrative and operational activities for ongoing quality improvement projects. Ensures that activities are carried out in accordance with established timeliness and specifications; coordinates interdepartmental functions in order to minimize project delays or course correction when necessary. Meets with key stakeholders on a regular basis to review project status and identify needs

Duties & Responsibilities

Provide project leadership. Assist in the design, training, and implementation of support activities for non-clinical and clinical staff. d develop future action plans.
Provide advice and counsel serving as a consultant to other staff, physicians and other stakeholders across the care continuum (internal and external).
Collaborate with medical center business partners, technicians, technology staff and vendors to define project scope, desired results and deliverables.
Create and update project documents, preserve and maintain key project information for future use and reference.
Assign individual responsibilities, and manage tasks to meet deadlines. Oversee deliverables assigned to colleagues, vendors, and others to ensure timely completion of projects.
Meet with team members on a regular basis to review project status, keep stakeholders (internal and external) informed of changes, and plan for future actions. Manages and facilitates team and user meetings. Communicates relevant updates to management and physician leaders.
Develop, drive and maintain a detailed project schedule with team input, including schedules, project milestones, metrics, work plans, resources, communication plans, issue resolution plans, and status reports. Identity project risks/gaps and create risk mitigation plan.
Represent Continuum of Care Administration/Department on various clinical and operational and improvement initiatives.
