Description:

Position Summary
This position provides paraprofessional assessment of patient needs and serves as a resource person for participants in the program. Assists in the preparation of patient records and works with nursing staff on guiding patients through the referral process and obtaining prior authorizations for medications as needed. The positions works under the general supervision of the Administrative Nurse II.

Duties & Responsibilities

Completes initial intake for new patients assessing needs and provides information to patient, and family if needed, regarding program requirements. Schedule patient and obtain a signed Release of Information for records, fax to appropriate source and monitor the receipt of those requested records. Communicate with staff regarding intake, and any special needs the patient/family may have. Maintain records on new intakes on the L Drive and Epic
Work with the provider and nursing staff on patient referrals and obtaining prior authorizations as needed for medications, testing and procedures
Serves as an initial point of contact for program questions/guidelines. Develop education materials and provide information to patient and family regarding community resources for all PHS programs
Collaborate with providers and nursing to co-ordinate patient care requirements such as faxing lab orders and following up to obtain results
Perform other related duties and participate in special projects as assigned

Qualifications:

Minimum Qualifications


Any one or any combination totaling two (2) years (24 months) from the following categories:
college course work in behavioral science, social work, sociology, psychology, or a closely related field as measured by the following conversion table or its proportional equivalent:
30 semester hours equals one (1) year (12 months)
Associate's Degree (60 semester hours) equals eighteen months (18 months)
90 semester hours equals two (2) years (24 months)
Bachelor's Degree (120 semester hours) equals three (3) years (36 months)
work experience that provided a knowledge of human behavior by having demonstrated through public contact the ability to cope with a variety of people, (such as customer service, salesperson, or receptionist).
