Position Highlights:
Position: Registered Nurse Transitions Navigator
Location: 120 Spalding Rd, Naperville, IL
Full Time 40 hours per week
Hours: Monday-Friday, M, W, F 7am-330p and Tues, Thurs 830am-5pm
Required Travel: No

What you will do:
Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served. This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs.
Provides education and information to the patient and family, helping to make the outpatient care seamless, continuous and comprehensive. Initiates and documents patient teaching including family and significant others based on assessment of needs. Reinforces education on disease, treatments, side effects, adverse reactions, and reportable sign signs/symptoms based on multidisciplinary plan. Utilizes appropriate patient education documentation modality.
Acts as the single point of contact for patients and families. Coordinates and schedules tests,procedures, appointments, and treatments as needed. Assists in the development of a treatment plan that is understandable and feasible to the patient. Acts as the point person for patients and all other care providers.
Supports the patient during difficult decision-making periods. Directs/refers patients and families to available supportive care services, including dieticians, social workers, financial counselors, and community resources.
Partners with patients, families, the interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate and exemplary, interdisciplinary care enhancing patient experience. Communicates with all members of the healthcare team about patient/family needs and concerns, and barriers as appropriate. Partners with other healthcare disciplines to ensure timely appointments, results reporting, and communication on patient care and follow up. Triages patient calls and functions as an RN in the care clinic.
Initiates and performs ongoing review of clinic processes related to service provided and provides to supervisor for approval. Updates or writes new policies to enhance professional practice where appropriate and provides to supervisor for approval. Develops and/or coordinate development of patient education materials, clinical pathways, and quality studies.
Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures as well as staff education with the Clinical Educator. Promotes community education and increases prevention awareness and early-detection screening programs.
Coordinates quality assessment and process improvement initiatives related to ensuring patient access to care.

What you will need:
Education, Experience and/or Skills Required:
Bachelor's Degree in Nursing or higher
A minimum of five years of previous experience in an outpatient setting
A minimum of three years of previous experience in patient education
Strong problem-solving skills
Ability to function in an organized and time conscious manner
Education, Experience and/or Skills Preferred:
Master of Science in Nursing
Knowledge of inpatient setting
A minimum of one year of case management experienceLicense/Certification Required:
Current State of Illinois Registered Nurse licensure
Current CPR certification issued by the American Heart Association or American Red Cross
