Job Description

SUMMARY              

Coordinate timely access and linkage to IHS sites per HealthLine Operations policies and procedures.  Oversee all aspects of health service resolution; responding timely to questions and/or directing callers to appropriate parties. Expected to be on call at least 2 weeks out of the month using an after hour cell. Provide excellent customer service.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

  • Provide excellent customer service for youth in care, caregivers, providers, community members and others, including, but not limited to:
  • Providing information regarding HealthWorks procedures and services.
  • Providing referrals to HealthWorks enrolled medical providers and specialists.
  • Ensuring the prompt notification of youth in care and caregivers of dis-enrolled physicians.
  • Recording information regarding the number, nature, and outcome of calls to the HealthLine.
  • Answer all after hour calls and document all necessary information when on-call
  • Assisting caregivers, clients and caseworkers to schedule appointments.
  • Assist with following up on CHEs and requesting medical records.
  • Ensure that 98% of youth in care are assigned a primary care physician.
  • Perform other duties as assigned.
