Job Description

This position will join the Heritage's Community Support Programs to provide support, engagement, assistance, transportation, and linkage to various resources, to adult with a diagnosis of mental illness. Services will be provided in array of settings including individual's home, community agencies, healthcare providers, and Heritage. This position is full time and works collaboratively with clients, community providers/organizations, and Heritage staff.
Supportive Housing Responsibilities Include:
* Home visits to assess client(s) in their home environment
* Providing transportation assistance to and from clinical treatment services, doctor or dental appointments, budgeting activities for groceries and/or personal items, and community activities
* Assistance to client with maintaining their home including cleaning apartment, move and apartment clean outs, and assembling new furniture for client's new apartments
* Coordination of bed bug treatment procedures
* Locating housing resources and communications with landlords
* Assistance to client in coping/waiting in hospital emergency department and/or pre and post-surgery
* Participating in clinical staffing of shared clients
Clinical and Case Management Responsibilities:
* Coordination and active participation in the referral and linkage to all healthcare including primary care, specialty care, testing and treatment for high risk conditions, mental health, dental, and vision
* Assistance with obtaining eligible benefits including Medicaid, Social Security, Link Cards and other entitlements
* Assistance with navigating how to support and assist clients with housing stability, employment, and education
* Assistance with developing positive supportive activities (i.e. relationships with friends and family members, support groups, wellness activities, or identifying/participating in positive social activities that support recovery)
* Assistance in engaging and linking individuals to accessing eligible transportation options
* Provide medication monitoring and training
* Participate in a team-based approach to providing treatment including collaboration with clinicians, nursing, healthcare providers, probation, landlords, and other partners invested in the individual's recovery.
* Attend clinical staffing(s) to collaborate with providers to coordinate treatment including nursing, physicians, clinicians, and the ACT Team.
Knowledge, Skills, and Ability:
* Ability to work with individuals with a diagnosis of mental health and/or substance use in a kind, caring, and supportive approach
* Knowledge of case management techniques
* Knowledge of community resources including psychiatric, substance use, medical, and/or other community services
* Effective oral and written communication skills
* Ability to work as a team player
* Strong customer service skills
* Ability to navigate electronic health record
* Proficient in Microsoft Word and Excel
Education and Experience: The ideal candidate will have experience working with individuals in behavioral health settings by providing a person-centered approach to support an individual's wellness and recovery. Key characteristics for this position include the ability to meet people where they are at, non-judgmental, and like a fast-paced position. GED or high school education, as well as a minimum of 21 years of age, are required.
Valid driver's license, reliable means of transportation, and proof of current automobile insurance required.
