Job Description

Heritage is seeking a dedicated and compassionate individual for a full-time Veterans Care Coordinator position to be housed on-site at our main Decatur, IL facility. This role involves working closely with veterans to navigate multiple service agencies and enhance collaboration to ensure their needs are met. As the primary coordinator, you will manage the day-to-day supportive program operations for veterans from entry to discharge. This position also focuses on fostering mental health, recovery, and wellbeing among veterans.

Responsibilities Include:
Empower veterans and their support systems to utilize and build their strengths, resources, and support to achieve goals independently.
Promote teamwork and collaboration among staff members assisting veterans.
Assist veterans and their families/natural supports in connecting to necessary community resources, including mental health, medical, financial, housing, educational, vocational, and social support services.
Develop positive working relationships and maintain routine communication with veterans, family members, the Veterans Administration, referral sources, service providers, and community resources to facilitate effective service delivery.
Help veterans develop and practice coping skills to manage mental health, substance use, and health-related symptoms.
Assist in obtaining eligible benefits, such as Medicaid, Social Security, Link Cards, and other entitlements.
Support veterans in developing positive activities, such as re-establishing relationships with friends and family, joining support groups, participating in wellness activities, and engaging in social activities that support recovery.
Aid in accessing eligible transportation options.
Complete all required paperwork accurately, legibly, and within established timelines.
Perform other duties as required within the scope of the position and the employee's experience, education, and abilities.
Education and Experience:
This position requires applicants to be a Veteran with willingness to share your experience to help others.
Heritage is seeking candidates with the following qualifications:
Experience in working with veterans or within veteran services is preferred.
Strong organizational and communication skills.
Ability to work collaboratively with a diverse team and community partners.
High School Diploma or Equivalent.
Valid Illinois Drivers License, reliable means of transportation, and proof of automobile insurance.
