GENERAL SUMMARY:

Reporting to the Clinical Director or Clinical Unit Leader, performs as a member of the Patient Care Team. Performs patient care tasks delegated by and under the direction of the Registered Nurse. Demonstrates the knowledge and skills necessary to assess, treat, and provide care appropriate to the age(s) of the patients served on his or her assigned unit. This job description was developed in accordance with the Illinois Nurse Practice Act and the ANA Scope and Standards of Practice.

REPRESENTATIVE FUNCTIONS:

Provides patient care under the supervision of a Registered Nurse (RN) and in accordance with the State of IL Nurse Practice Act based upon the developmental stages of the patient: Assists RN in patient care activities and tasks, as delegated. Administers medications as delegated by the RN and within their scope of practice.

Collects and documents patient monitoring data. Observes, documents, and reports changes in patient's condition/environment to the RN. Documents changes in existing Care Plan after collaboration with RN.

Communicates effectively with patients, and members of the health care team. Conveys information regarding patient care needs to appropriate health care team members. Interacts positively and therapeutically with patients and family members based upon the developmental stages of the patient.

Shares responsibility for care and maintenance of equipment and supplies on unit.

Participates in and supports continuous quality improvement activities.

Adheres to all Northwest Community Hospital standards, policies, and procedures.

SPECIAL SKILLS AND ABILITIES REQUIRED:

The interpersonal skills necessary to interact effectively with patients, physicians, and other hospital personnel.

The organizational skills necessary to prioritize multiple tasks and complete them in a timely manner.

The written communication skills necessary to document patient information.

KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

Graduate from a state accredited School of Practical Nursing.

Current LPN license from the state of Illinois.

Current CPR certification through the American Heart Association.

Minimum 1 year experience as an LPN preferred.

Knowledge of and/or experience in the correct use of body mechanics.

PHYSICAL REQUIREMENTS:

Requires continuous standing and walking for most of shift. Requires frequent (3-6 times per hour) bending, stooping, squatting, kneeling, and overhead reaching and frequent lifting and carrying of 15 lbs. to 30 lbs. Requires occasional (10-15 times per day) lifting and carrying of 30 lbs. to 70 lbs. and occasional pushing/pulling of up to 300 lbs in wheeled equipment.

Flow of work requires concentrated mental, visual, and auditory attention most of the time. Concentrated mental, visual, and auditory attention combined with manual dexterity is required for sustained periods of time.

Requires visual acuity adequate to assess patients and read/record patient charts.

Requires auditory acuity adequate to converse with patients, take phone orders, and receive directions from physicians and coworkers who are wearing masks.
