Summary

Rush University Medical Center’s Road Home Program: The National Center of Excellence for Veterans and Their Families (RHP) is dedicated to assisting and supporting current service members (active duty, reserve, and national guard), veterans, and their families with effective clinical services focused on treating mental health diagnoses. All services are provided at no out-of-pocket cost to the client. RHP has an outpatient clinic that provides in-person and virtual services such as individual, couples, family, child, and group psychotherapy as well as medication management and neuropsychological testing. RHP also specializes in the treatment of posttraumatic stress disorder (PTSD) utilizing a 2-week in-person intensive outpatient therapy program (IOP) for service members and veterans from across the U.S. as well as a virtual accelerated treatment option for those with barriers to in person attendance.

The Patient Care Navigator (PCN) is the first point of contact for all clients seeking mental health care at RHP and thus a vital role and member of the RHP team. PCNs register patients for services, assist them with navigating the electronic health record system, help obtain medical records, and schedule initial appointments. They assist clinicians with case management duties and are a critical part of the team that manages logistics prior to an individual attending the IOP. Individuals in this role will manage a large amount of patient data and thus must be organized and able to track a caseload of patients. The PCNs maintain regular contact with all individuals accepted to the IOP, conduct suicide and substance use coping plans, and conduct outreach calls once clients complete programming. RHP serves a broad array of active duty military personnel and veterans with minoritized ethnic or racial identities, LGBTQ+ identity, low socioeconomic status, and other minoritized identities. So, demonstrated experience with one or more of these factors is desired. RHP strives to hire staff who reflect the diversity of the population we have the honor to serve.

Responsibilities

· Answering main phone line, checking voicemail, checking RHP email, and follow-up contact to register individuals for services.

· Register individuals for services utilizing Epic electronic medical record system, setting up MyChart access, and scheduling new patient appointments.

· Case management duties included but not limited to faxing documents, obtaining and sending ROIs, mailing letters, contacting providers to obtain medical records, sending documents to Rush’s health information management department, facilitating referrals to partner organizations for financial assistance, and communicating with external providers regarding clients’ status in the program.

· Conducting regular contact with all patients who are accepted for the RHP IOP and informing the multidisciplinary team about logistics or changes in attendance.

· Conducting outreach calls with all patients who have completed the RHP IOP to assess any additional case management needs.

· Tracking information of all individuals who contact RHP for services as well as information regarding Patient Care Navigators attempts at outreach and scheduling.

· Understanding and keeping track of all of the services at RHP with which a potential client may seek to engage.

· Document all patient correspondence in electronic medical records as necessary.

· Participate in meetings as assigned.

· Other duties as assigned

Other information:

Required Job Qualifications:

- High School diploma or GED and 1 year in experience Electronic Medical Record

- Knowledge of ambulatory setting

- Basic knowledge of Microsoft Office

- Written, verbal and exceptional interpersonal communication skills

- Ability to multi-task

- Ability to function independently, to organize, and be self-motivated

Preferred Job Qualifications:

- Associates degree (health related area of focus such as social work, psychology or medical terminology)

- Bilingual
