POSITION TITLE: Senior & In-Home Services Program Supervisor

REPORTS TO (TITLE): Senior Services Program Manager

DEPARTMENT MISSION
To assist in accomplishing the mission of The Salvation Army through the coordination of all mission activities, corps and social service, within the City of Chicago.
OUTCOMES
Supports the full mission of The Salvation Army in providing the highest quality of in-home services to families and the elderly. Supervises and provides leadership for the Grandparents and Older Relative Raising Children program and for the CAS program. Develops collaborative partnerships with other Salvation Army units and community partners in order to achieve an integrated model of mission for the program.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Administration
Provides administrative oversight to the Grandparents and Older Relatives Raising Children program (GORRC).
Provides assessments to determine to needs of GORRC families and provide them with the appropriate services.
Provides, within agency policies, programs, and procedures, casework services to individuals and families.
Conducts support groups related to parenting and family issues.
Conducts educational programs for seniors in GORRC program.
Assists families with GAP Filling Funds that complement the care provided by the older relative.
Works collaboratively with partners as well as developing new partnerships within The Salvation Army and the community to provide resources for participants.
Maintain, in a timely manner, required documentation of participant's involvement including but not limited to assessments, demographic data, case notes, closing summaries, and outcomes data collection.
Performs well-being checks when assigned.
Program Administration.
Collect, input information into data base and monitor billing.
Submits reports for funding sources and The Salvation Army North and Central Illinois Division.
Obtains appropriate Authorization for Releasing/Obtaining Information before confidential client information is given out or requested from another source.
Ensures that the program case recordings, data, forms, and reports are completed according to policies and procedures.
Conducts assessments of services.
Abides by Codes of Ethics.
NASW, including standards for cultural competence in social work practice.
The Salvation Army Social Services Code of Ethics.
Supervision
GORRC
Coordinates, assigns and supervises staff to fulfill assigned cases.
Provides oversight for all cases. Accepts new cases referred from contracted referral sources and completes intake forms.
CAS
Coordinates, assigns and supervises staff to fulfill assigned cases.
Provides oversight for all cases. Accepts new cases referred from contracted referral sources.
Initiates contact as a mandated reporter.
The Illinois DCFS when child abuse or neglect is suspected.
Adult Protective Services when elder abuse or neglect is suspected.
Maintains awareness of applicable liability issues and other applicable legal issues, and their potential to affect agency and/or clients.
Keeps Manager and Director informed regarding case related subpoenas and attends court when appropriate.
Intra-Agency Responsibilities
Collaborate with other Salvation Army units and personnel to successfully maintain and achieve policies and procedures, goals and objectives, and outcome-based measures related to City Mission.
Attends agency consultations, staff development programs, team meetings and bi-weekly supervision/consultation with the Manager.
Understands, and can effectively work under, the mission of The Salvation Army and core values/objectives of City Mission.
Community Relations
Represents the agency in the community to promote greater understanding of programs.
Attends community meetings, as requested by the Manager.
Maintains relationships with contractor staff for referrals, problem solving and community relations.
Provides information to public and Salvation Army programs about services, when requested.
Markets programs within the community and The Salvation Army.
Planning/Evaluation
