Job Description

GENERAL SUMMARY:

Reporting to the assigned Unit Director, coordinates matrix relationships that involve collaboration with multiple healthcare team members in various healthcare settings. Provides consultation and support to the nursing staff, physicians, and other team members on patient resource and psychosocial matters and concurrent review issues upon request. Identifies community resources, coordinates financial and clinical aspects of health care, and counsels patients on health care and treatment options. Maintains accountability for process improvements that reflect increased quality and decreased cost of care.

REPRESENTATIVE FUNCTIONS:

Demonstrates the knowledge and skills necessary to assess and care for the assigned age groups(s) of patients. Implements assessment and care based on the developmental stages of the patient. Involves family/guardian in the plan of care and conducts patient and family education, as appropriate, considering the developmental needs of the patient and the impact of social/behavior issues on the illness and recovery process.

Provides direct social work service and counseling to patients and their families and/or groups of patients in relationship to emotional problems, social and practical environmental problems, and family and group therapy/counseling /support.

Identifies strengths and resources available to the patient and family; documents recommended actions, interventions or referrals (within the Social Services evaluation summary in the Mental Health.Network only).

Collaborates with interdisciplinary teams to coordinate aftercare planning for patients.

Identifies high risk, problem populations and intervenes to coordinate care for these patients in treatment. Continues to serve as a resource to those identified populations after discharge to promote health and access to treatment. Collaborates with interdisciplinary team in interactions with complex community agencies. Provides crisis intervention assessment, and mandated reporting in cases of domestic violence and elder and child abuse.

Collaborates with key resources both in the community and within the NCH continuum as a patient advocate.

Adheres to all Northwest Community Hospital standards, policies, and procedures.

SPECIAL SKILLS AND ABILITIES REQUIRED:

The interpersonal and collaborative skills to establish and maintain effective liaison with patients families and other hospital personnel as well as external customers (i.e., payers, reviewers, skilled nursing facilities, home health agencies, and other community support resources) and consistently apply the principles of excellent customer service.

The organizational skills necessary to work independently and anticipate, prioritize, and follow through on daily activities.

The decision-making skills necessary to work independently but seek consultation when appropriate in the context of a matrix reporting environment.

The ability to work effectively in a team environment with other healthcare providers and support staff demonstrating NCH core accountabilities of teamwork, respect, and caring, and effective use of resources and initiatives.

The written and verbal skills necessary to communicate effectively with patients, families, physicians, and staff and write appropriate continuity of care notes.

The practical problem solving skills to deal with a variety of concrete and abstract variables in situations where creative solutions are required.

KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

Master's Degree from an accredited school of Social Work.

Licensure to practice social work in Illinois (LCSW) or LCSW eligible.

Minimum of two years recent clinical experience in the social work field in an acute care setting; mental health setting and/or medical social work experience preferred.
