Job Description

GENERAL SUMMARY:

Reporting to the Director of LOMG Counseling Services and/or LOMG Practice Manager, provides quality clinical therapeutic services to patient populations and their families/significant others in a physician office or other settings (NCH/MG). The Clinical Therapist serves as a member of the interdisciplinary team, supporting the organization's treatment program and philosophy assuring the delivery of quality treatment. Orienting and mentoring new staff to the program.

REPRESENTATIVE FUNCTIONS:

Demonstrates the knowledge and skills necessary to provide clinical therapeutic services appropriate to the age of assigned patient population. This includes knowledge of the physical and psychological needs of the patients as well as the ability to respond appropriately to those needs.

Conducts psychosocial assessments by interviewing the patient and their significant others to obtain history and identify preliminary issues for treatment focus.

Contacts appropriate referral sources and/or previous or current tertiary care providers to inform of client's enrollment in treatment, obtain relevant collateral clinical data and coordinate care.

Develops implements and monitors progress of the patient's individualized treatment plan.

Provides individual, couples and family process-oriented counseling sessions as appropriate using various professional treatment modalities.

Participates in regular clinical team meetings and provides input regarding case discussions on the psychosocial, disease management and case management needs of patients.

Demonstrates an understanding of, and adherence to, all procedures and legislation which apply to patients cared for in a Mental Health and/or Substance Abuse treatment setting.

Informs attending physicians (Physicians, NP's, etc.) in the incidence of psychotropic medication management, of client's treatment progress and/or significant changes in emotional/mental status, which undermine client's daily functioning.

Petitions clients and effectively transitions them to inpatient care in the event of the need for psychiatric hospitalization where applicable.

Demonstrates a comprehensive knowledge of community resources (12 step groups, support services, educational resources, etc.) to assure appropriate supplemental care or follow up services after termination.

Demonstrates a comprehensive knowledge of community resources (inpatient treatment settings, recovery homes and residential treatment facilities) and medical allied staff expertise to assure appropriate transition to elevated levels of care for clients requiring more intensive treatment services.

Maintains accurate and timely medical records including psycho-social assessments, family sessions and daily progress notes. Provides descriptions of therapeutic interventions and patient response for discharge planning process.

Participates in activities that enhance professional growth and development and demonstrates a willingness to participate in committees to improve clinical care

SPECIAL SKILLS AND ABILITIES REQUIRED:

The critical thinking skills necessary to assess, evaluate, draw accurate conclusions, and make decisions based on factual evidence.

The leadership skills to effectively lead, motivate, discipline, communicate sensitive information, coach and mentor staff.

The interpersonal skills necessary to interact with patients, families, hospital personnel, physicians, and community agencies.

Ability to maintain information which is highly confidential and to clearly summarize pertinent clinical information via written correspondence and medical records documentation.

Ability to demonstrate self-control in volatile or hostile situations, and set therapeutic limits to maintain therapeutic distance with clients; maintains therapeutic boundaries with patients during treatment and following termination.

The organizational skills necessary to effectively manage time and handle multiple priorities.

KNOWLEDGE, PRACTICAL EXPERIENCE AND LICENSURE/REGISTRATION REQUIRED:

Master's degree in Social Work, Psychology or related program required, Psy D. preferred.

Minimum of two years of experience in a behavioral health setting.

Current State of Illinois License with a minimum of four letters which may include LCSW, LCPC or LMFT required.

Current CPR Certification issued by either American Heart Association or Red Cross.

CPI (Crisis Prevention Intervention) certification within 90 days of hire.
