Position Overview

The Illinois Department of Children and Family Services is seeking an organized, professional, and results oriented individual to serve as Clinical Placement Facilitator.  Subject to management approval, this position will coordinate, monitor, and develop the clinical placement program for an assigned geographic area.  The position will develop and implement a plan to facilitate the achievement of living arrangements at an appropriate level of care to meet the needs of individual clients and will develop and implement a localized plan to identify resource needs for living arrangements.  This position provides a great opportunity for someone who is passionate about children and interested in playing a role in ensuring that placements at the appropriate level of care are provided for children and youth.  DCFS offers a competitive compensation plan, excellent benefits, and a pension program.  The ideal candidate for this position will have excellent organizational skills and strong communication skills.  We invite you to join our innovative team to help make a positive difference in the lives of the children and families of Illinois.

Job Responsibilities
  1. As a Clinical Placement Facilitator, conceptualizes, coordinates, monitors, and develops the clinical placement program for an assigned geographic area
  2. Coordinates and provides professional review of referrals for all levels of care placement services to ensure referrals are appropriate and include a complete statement of intended purpose of the identified level of care, the client’s specific treatment needs, projected duration of treatment in the level of care treatment services, and an initial treatment discharge date
  3. Coordinates the collection of data in support of a system to track referrals to placement providers and determine outcomes
  4. Maintains a working knowledge of all residential/group home/independent living/transitional living/foster care placement programs under contract with DCFS
  5. Maintains and monitors program plans for levels of care service delivery
  6. Provides input into the monitoring of facility utilization
  7. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above
