Overview

The PACE position is ideal for someone who is adaptable, versatile and motivated to enhance an OSF start-up program. The Program for All-Inclusive Care for the Elderly (PACE) is new to OSF and presents an excellent opportunity for individuals who are motivated and want to help define a new care model. PACE responsibilities will include regular care conferences, group and one-on-one clinic interventions, home based care and overall management and companionship for enrolled seniors. The PACE center is located downtown Peoria.

Starting pay for this position is $18.43/hour; base pay offered may increase based on experience.
POSITION SUMMARY:
The PACE Patient Care Technician/Certified Nursing Assistant (PCT/CNA) provides appropriate safe basic care to participants which includes taking vital signs, assisting participants with activities of daily living, specimen collection, and observation of participants for physical/emotional changes. Other duties include recording pertinent information in participant charts, communicating with other Participant Care Team members to ensure continuity of care, and may function in a role providing constant supervision to participants as needed. Duties may vary according to department expectations and needs.
Qualifications

REQUIRED QUALIFICATIONS:
Education:
High School Diploma/ GED
Experience:
1 year of experience working with a frail or elderly population or, if the individual has less than 1 year of experience but meets all other requirements, must receive appropriate training from the PACE organization on working with a frail or elderly population upon hiring.
Licensure/ Certification:
CNA- Certified Nursing Assistant
CPR American Heart Association HealthCare Provider Basic Life Support (BLS) required before providing patient care.
Maintains a valid driver's license.
Other Skills/ Knowledge:
Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Strong analytical and problem solving skills, with the ability to be detail oriented.
Provides own transportation and auto insurance and abides by all laws, rules and recommendations for safe driving while on duty.
