Position Highlights:
Position: Case Manager, Behavioral Care Partners
Location: Corp Center/Warrenville
Full Time
Hours: Days
Provides quality case management to patient population and their families/significant others. In collaboration with the physician and other health care team members, the Case Manager coordinates the patient's care to achieve optimal patient outcomes. Serves as a member of the interdisciplinary team, supporting the organization's treatment program and philosophy assuring the deliverance of quality treatment.
What you will need:
Required Education and/or Experience:
Master's Degree in behavioral health field or a nursing degree
Minimum five years psychiatric clinical experience
Excellent communication skills

Preferred Education and/or Experience:
Previous managed care experience strongly preferred
Previous case management experience
Previous supervisory experience
Discharge planning experience
Master's Degree in Psychiatric Nursing, Social Work, Counseling or a Doctorate in Psychology

Required License and/or Certification:
If RN, current State of Illinois licensure
LCPC (Licensed Clinical Professional Counselor), LCSW (Licensed Clinical Social Worker), LSW (Licensed Social Worker) or LPC (Licensed Professional Counselor)
