Thresholds is seeking a skilled, energetic, and flexible Team Leader for our Youth Transitional Living Program's case management team. This program consists of REACH, a youth transitional living program (TLP), and the Mothers Project, a TLP that serves pregnant and parenting young mothers. Both TLPs offer comprehensive support to young people between the ages of 17 and 24 who are experiencing mental health challenges through residential support, case management, and therapy services. The Team Leader is responsible for the direct supervision of all the members of the case management team in order to insure quality service delivery to REACH and Mothers Project members and functions as the senior clinician or "player coach" of the team.
ESSENTIAL DUTIES & RESPONSIBILITIES
Assumes the leadership role within the Youth Transitional Living Program's case management team. Models evidence-based practices and recovery to ensure team adherence and understanding to fidelity scales. Ensures that 60% of services are provided in the member's natural setting.
Gathers member information and assesses the need for service, makes recommendations for treatment, identifies and investigates existing natural resources and supports, explains options available to the member and assists in the application process.
Functions as a DCFS Supervisor, overseeing Case Managers who function as DCFS Case Workers.
Actively leads and oversees the activities of team staff members providing outreach, case management, social, vocational, education, physical well-being, and programming and other services and supports provided by the treatment team.
Meets required productivity targets and completes all documentation within assigned time frames. Monitors and coaches staff on meeting productivity measures.
Monitors member attendance and/or occupancy on the team so that case assignments, transitions and closures are handled effectively and efficiently.
Reviews all required documentation. Maintains the quality, validity and confidentiality of individual case records according to DCFS, CARF, Medicaid standards and agency traditions. Assures that all assigned documentation is completed accurately, on time and is organized in a systematic manner.
Ensures that Case Managers are in compliance with court orders. Assists Case Managers in court as needed.
Ensures that all documentation for court is thoroughly completed and on time to comply with DCFS and court requirements.
Responsible for bi-annual court reviews and documentation.
Assists with special tasks as assigned, including but not limited to: representing the agency at court hearings, meetings, conferences, supervising students/interns, orienting and training new staff.
Conducts annual evaluations in a timely manner for all team staff. Develops specific goals and objectives, and provides the method and opportunity for each team staff to succeed in their professional development.
Assesses and makes recommendations to ensure homes for safety in compliance with CERAP.
Participates in the hiring sequence of the agency, and make recommendations on the hiring or termination of team staff.
Follows agency standards regarding clinical supervision. Assists program leadership with the development and monitoring of Performance Improvement Plans for staff not consistently meeting agency standards.
Other duties as assigned.
EDUCATION & EXPERIENCE
Master's degree in Psychology, Counseling, Social Work, or related field required
LCPC, LCSW, or Licensed Clinical Psychologist preferred
CWEL license and Therapeutic Crisis Intervention (TCI) certification required (both can be obtained through Thresholds after hire)
Experience providing case management to youth with DCFS involvement preferred
Supervisory experience preferred
ADDITIONAL REQUIREMENTS
Flexibility with schedule for on-call rotation and occasional evenings.
Current and valid driver's license
Daily access to a personal vehicle
Willingness to transport program participants
