Summary:

The Legacy Mental Health Fellow (Care Coordinator LP) is a key member of Rush University System for Health (RUSH) and must exemplify the Rush mission, vision and ICARE values and act in accordance with RUSH policies and procedures, including complying with all RUSH University Medical Center Customer Service Standards, and represents a collaboration between the Garfield Park Rite to Wellness Collaborative, RUSH University Medical Center, and Chicago State University’s MSW Program.

The fellowship also represents an effort to reduce health disparities by addressing mental health treatment access and improvement as an integral aspect of overall health and well-being of West side residents. This position will receive clinical supervision and training to build towards their LCSW licensure hours in an interprofessional setting with hospital based and community-based components. The fellowship’s values are in providing trauma and culturally informed care, community-based practice, patient-centeredness, and strengths-based approaches.

Responsibilities:

- Assess psychosocial needs by phone, in-person and/or in community-based settings to provide necessary support to individuals within Rush and in neighboring communities.

- Communicate with client/patient and family to offer information and assistance, available resources and care coordination.

- Assists patients/caregivers, and community members navigate the system by removing potential barriers to support and care.

- Promote development of Rush programming which places support and educational activities alongside outstanding care management services to maximize impact, external recognition and growth in coordination with other departments.

- Actively plan for and participate in supervision spaces.

- Learn/understand context of current healthcare policies as it relates to Rush as an academic medical center and the communities surrounding the hospital.

- Increase participation in Rush wellness programs such as Rush Generations and other supports that increase access to health education and wellbeing.

- Promote Rush University Medical Center clinical services by referring clients to relevant Rush service lines.

- Connect and liaise with other departments to develop mutually beneficial alliances.

- Actively seek out internal and external opportunities for growth to move toward obtaining social work licensure

Required Job Qualifications:

- Bachelor’s Degree in Social Work

- Understanding of older adults, family caregiving, chronic/debilitating disease/disabilities, and social determinants of health (SDoH) issues.

- Strong assessment and care management skills.

- Demonstrates initiative and strong interpersonal, teamwork and communication skills.

- Computer literate.
