Overview

Expected pay for this position is $21.30/hour. Actual pay will be determined by experience, skills and internal equity.
This job is located in Peoria,IL.
POSITION SUMMARY: The Sickle Cell Community Health Worker (CHW) helps improve care delivery for current pediatric and adult sickle cell patients and their families. They assist with the transition of adolescent and young adult sickle cell patients from pediatric to adult care and will work to coordinate appropriate hospital and community resources to ensure access to care, including setting up transportation for patients along with assisting with non-clinical needs in the infusion center.
Qualifications

REQUIRED QUALIFICATIONS: Education: High School/GED Experience: 1 year of experience (including volunteer) with patient education, health education and/or in supporting behavior change relative to chronic disease management. Licensure/ Certification: American Heart Association BLS certification within 6 months of hire. Community Health Worker Certification must be obtained within one year of hire. Sickle Cell Community Health Worker Certification must be obtained within two years of hire. Other Skills/ Knowledge: Excellent interpersonal and communication skills. Solid computer skills, including proficiency with Microsoft software. Ability to effectively communicate and work with patients in an informal setting, including ability to listen to patients, respond non-judgmentally, be flexible, and deal with complex situations. Knowledge about cultural competency in healthcare and diversity, equity, and inclusion (DEI). Strong analytical and problem-solving skills, with the ability to work independently and be detail oriented. Ability to learn and apply basic instructions on a regular basis.
PREFERRED QUALIFICATIONS: Education Associate's degree in healthcare related field Experience: N/A Licensure/ Certification: N/A Other Skills/ Knowledge: Knowledge of sickle cell disease across the lifespan. Knowledge of social determinants of health
