Schedule: Part Time

Shift : 16 hours/week; Tuesdays and Thursdays 8:00am – 4:30pm

Location: AdventHealth Glen Oaks Hospital – Medication Therapy Management Clinic(MTM)

The role you’ll contribute:

As a Clinical Specialty Pharmacist for the MTM clinic , will conduct in-person and virtual patient visits in an independent pharmacist led chronic disease management clinic. Work under a collaborative practice agreement to adjust medication therapy plans to achieve disease state goals.

Assures the appropriate selection, dosing, administering, and dispensing of medication orders in specialized areas, such as diabetes, heart failure, and anticoagulation. Serves as a subject matter expert in pharmacy discipline and provides guidance across pharmacies and to clinical leadership.

- Reviews and assesses medication orders to identify, prevent and resolve potential and actual drug related problems.

- Recommends formulary alternatives when appropriate.

- Provides drug information and education to medical staff and patients.

- Communicates with physicians and other clinicians as necessary to facilitate care.

- Oversees technical personnel who assist with medications and supplies.

- Assists with academic functions such as serving as a preceptor for pharmacy students and/or pharmacy residents. May float to provide coverage of pharmacy needs.

- Provides patient counseling when required.

Qualifications

The expertise and experiences you’ll need to succeed:

EDUCATION AND EXPERIENCE REQUIRED:

Bachelor’s degree in Pharmacy or Doctorate of Pharmacy required

EDUCATION AND EXPERIENCE PREFERRED:

Pharmacy residency preferred

LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:

Pharmacist credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.

· Board Certification preferred
