In our Youth and Family Services, we help parents and young people who are striving to become healthy and restore their relationships. We work in partnership with human services, child welfare systems, juvenile courts, and community-based service providers so that together we can help individuals and families obtain the services and skills they need to live in positive, healthy ways.


We are currently looking for full-time – Medical Case Manager

Starting at $40,000 – contingent upon experience, education, etc.

Provide interim medical case management to all DCFS Wards in the Lead Agency's service area through the first 45 days of custody.

  • Contact substitute care giver quarterly to get updates on doctor appointments, specialty doctor visits, or emergency room visits.
  • Request all medical Records two months before the Annual Case Review.
  • Insure all children in DCFS care are up to date on dentist, vision, and hearing appointments.
  • Insure all children in DCFS care maintain a primary care physician.
  • Assist substitute guardian with obtaining services or resources such as WIC.
  • Complete an assessment with substitute care giver twice a year to make sure the child (ren) in DCFS care are emotionally and physically stabled.
  • Provide a Health Passport to the care giver when needed.
  • Complete an Annual Case Review report for every child in DCFS custody
  • Maintain a network of primary care physicians that meet Maternal Child Health Primary Care Provider standards.
  • Maintain a positive relationship with agencies and case managers contracted by DCFS
  • Maintain records and keep current the name and address of the primary care physician, selected by the substitute caregiver or ward, for all eligible DCFS wards in Lead Agency's area (SACWIS).
  • Maintain a healthcare delivery system which has both the capacity and the capability to provide comprehensive EPSDT program health care services to DCFS wards in the Lead Agency area.
  • Develop and maintain written procedures to inform relevant parties of the specific actions which are required to ensure DCFS wards receive timely access to services.
  • Facilitate coverage when needed for other case managers
Requirements:

  • Bachelor's degree in social work or a related field with two years' experience in community health or maternal and child health.
  • Good communication and organizational skills.
  • Specific knowledge necessary for working with special population
  • Possess clinical skills to effectively manage clients
  • Knowledge of the Health Insurance Portability and Accountabilit
