Position Summary: The Case Manager Employment Specialist will provide support to youth and families by collaborating with other community resources to assess, planning, facilitate, and coordinate services to meet the youth and family’s needs. In this role, you will provide educational and employment assistance to adults recovering from trauma due to community violence. Working At NYAP

Responsibilities: The Reimagine Case Manager position may include the following, as well as other duties assigned:

+ Performs all work in a manner consistent with the National Youth Advocate Program’s mission, values, and philosophies.

+ Provide advocacy services and support to RVPS participants (at risk youth, justice involved youth and survivors of community violence).

+ Achieve successful employment outcomes.

+ Manage referral relationships with partner agencies.

+ Assess vocational functioning and make updates on an ongoing basis.

+ Develop job leads and assist with job searches.

+ Provide individualized, time-unlimited follow along services.

+ Educate and support employers, as permitted by the participant.

+ Ensures the consistent implementation meaningful employment workshops and programming for participants.

+ Assists individuals in identifying competitive positions that they are interested in applying for and reaching those goals.

+ Conductors outreach and networks in the community to identify potential places of employment for participants.

+ Prepares, revises, documents, and implements integrated service plans for each assigned program participant.

+ Ensures environmental support and assistive devices are available to promote independence.

+ Administers training for program participants.

+ Contacts employers after placement to ensure satisfaction with program participant’s performance.

+ Participate in required meetings.

+ Actively communicate with participants and families.

+ Assists at various outreach activities.

+ Acts as an advocate for Agency services and program participants.

+ Assess immediate crisis needs and long term needs of participants and address appropriately by providing services and service referrals ensuring stability.

+ Complete and maintain client records that document services provided.

+ Educate clients about dynamics of violence and assault and options/resources available.

+ Work with clients to develop and maintain relevant and meaningful case plans.

+ Provide concrete case management and access to resources.

+ Meet regularly with clients to support case plan accomplishment.

+ Refer clients to other agencies and resources and advocate directly with other agencies as needed to help client access necessary services.

+ Conduct assessments and intakes.

+ Facilitate weekly support groups for participants.

+ Performs other duties as requested.

Minimum Qualifications:

+ Bachelor’s degree in social work or human services.

+ Two years of experience providing case management services and care coordination.

+ Experience providing victim services, outreach services, community programs, criminal justice or related experience.

Other Skills:

+ Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet and database programs.

+ Significant experience in providing job skills training or post-secondary supports to high school students and/or young adults.

+ Strong written and verbal communication skills.

+ Excellent time management and organizational skills.

+ Excellent interpersonal skills and ability to interact and communicate effectively.

+ Communicate effectively with people from diverse cultures and backgrounds and knowledge of community resources and community organizations while maintaining a high degree of sensitivity, tact, and diplomacy during stressful situations.

+ Knowledge of needs and challenges experienced by people from diverse socio-economic and cultural backgrounds.

+ Knowledge of Criminal and juvenile justice systems and Victims’ rights within the criminal justice system.

+ Maintain records and documents and prepare accurate reports and case summaries while maintaining confidentiality with sensitive information.

+ Establish and maintain effective working relationships with those contacted in the course of work.

Driving and Vehicle Requirements:

+ Valid driver’s license

+ Reliable personal transportation

+ Good driving record

+ Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
