JOB SUMMARY: NNOC - MEMBERS ONLY
Under the direction of the Patient Services Director the Nurse Clinician performs all the functions of a Clinical Nurse II, in addition demonstrates a greater level of competency, knowledge and skill in delivering patient care.Provides comprehensive care for patients with multiple health problems. Participates in educational programs for staff, patients and families; coordinates clinic orientation and clinical practicum for new employees and assigned students.Participates in multi-disciplinary patient care conferences, committees, and QA/I activities.Functions as a resource for clinic personnel and other health care workers.
TYPICAL DUTIES:
Demonstrates competence in providing comprehensive care for patients, including those with complex health problems
Assists and or directs activities of other nursing personnel who participate in providing patient care
Assists staff in planning, developing, implementing and evaluate patient care activities
Provides guidelines and direction during emergency or crisis situations
Demonstrates knowledge of policy and procedures regarding delivery of patient care, safety, infectious disease, and patient care standards
Demonstrates knowledge and skill in assisting and or developing a comprehensive age-appropriate plan of care that consistently addresses the biophysical, psychosocial and environmental needs of patients, families and significant others
Investigates patient care problems and provides appropriate recommendations timely
Collaborates with staff, patients, significant others, and other disciplines in the needs assessments
Makes certain staff provides care that is consistent with the therapies of other disciplines, based on patient need and nursing practice standards, with consideration of patient's cultural and spiritual values
Participates in educational programs for staff, patients, and families
Assists in identifying the educational needs of staff, patients and families
Develops and coordinate clinic in-service programs for nursing personnel in collaboration with clinical specialists
Conducts in-services or coordinates training for new clinical equipment and medical supplies (e.g. point of care testing)
Coordinates clinical practicum and clinic orientation for all new employees
Coordinates student rotations to ensure maximum learning experience
Coordinates, develops, and maintains clinic-based orientation and competency checklists, reviews and revises as needed.
Ensures safe. effective. concise and timely communication amongst necessary staff and or disciplines
Participates in QA/QI activities and incorporates outcomes to enhance the quality of care in clinics
Participates in System wide QA committees and activities as needed
Maintains professional competency and leadership skills
Maintains current knowledge of licensing and regulatory agency requirements and practice standards
Provides education related to clinical practice to community liaisons (nursing affiliations, community groups etc.)
Demonstrates knowledge of CORE Center and System wide health care services and makes appropriate referrals as indicated
Demonstrates excellent internal and external customer service skills
Demonstrates awareness of patient and family rights, confidentiality, ethical and legal issues
Demonstrates responsibility and accountability for professional practice, growth and development
Adheres to CORE Center attendance policy
Adheres to universal and isolation precautions
Completes CCHHS annual online training timely (Infection control, hand hygiene, environment of care, patient safety, corporate compliance, HIP AA, fall/injury, restraint usage, recognition of abuse and neglect, recognition and treatment of pain, patient consent, communication/handoff, and other training as assigned)


MINIMUM QUALIFICATIONS
Current license as a Registered Professional Nurse in the State of Illinois is required.
