JOB SUMMARY: NNOC - MEMBERS ONLY
Under general direction, performs a variety of specialized and responsible nursing duties related to the care of patients in a large public health institution; and performs related duties as required. Assists in the development, implementation and monitoring of educational programs and in-services. Possess excellent clinical skills. Functions as a preceptor, participates in Quality Assurance projects, Department of Justice (DOJ)/NCCHC preparedness teams and CCH/Cermak committees.
TYPICAL DUTIES
Demonstrate clinical proficiency in assessing, planning and implementing patient care, taking into consideration the flow of the clinic and age-specific needs of the patient.
Plan, organize, direct and monitor orientation of nursing staff in collaboration with Nurse Coordinator/Nurse Educator/Divisional Director/Director PCS.
Works closely with all staff on meeting the requirements for the nurse sick call.
Lead DOJ preparedness teams.
Works with the Nurse Coordinator on the flow of the clinic and on ways to improve the Patient Experience.
Serves as a preceptor and resource for all services and initiatives.
Develop, plan and implement in collaboration with Nurse coordinator/Nurse Educator; educational needs of patient.
Works with the nursing staff to implement a plan of care for the patients that includes nutritional, educational, functional, social support, discharge teaching and psychological issues.
Attends ongoing meetings and trainings at various locations, such as Cerner updates and brings information back to the clinics to ensure staff are competent of the changes and updates.
Monitors staff to ensure required elements are documented correctly and works with staff to ensure an understanding.
Trains staff to electronically document appropriate information related to all patient encounters, using applicable Power Forms and I-View.
Trains nursing staff including RN's, LPN's, ERT's, CMT and PCAs.
Evaluate the plan of care in terms of expected outcomes.
Incorporate data derived from Care Management Tools into patient's plan of care.
Help patient establish and work towards meeting self-management goals.
Track patient's self-management progress.
Establish a therapeutic nurse-client, nurse-family, nurse-community relationship.
Schedule and conducts nurse visits based on the health services request form.
Evaluate and document outcomes of patient teaching.
Functions as a staff nurse in the event of staffing shortage.
Assess and document patient health literacy.
Ability to identify, monitor, implement Quality Improvement measures.
Make appropriate and timely decisions in a crisis or emergency situations.
Works and communicates effectively with a variety of individuals.
Delegate, coordinate and monitor patient care given by other levels of nursing staff.
Works closely with Nurse Coordinator on staff that may need additional support and develop an educational plan if needed.
Apply knowledge of standard operating policies and procedures.
Skill in the use/operation of clinical equipment necessary for providing patient care in the health center.
Teach, perform and/or direct Basic Life support.
Apply principle of infection control and safety.
Apply knowledge population-based data to health center patient assessments, care coordination and program development.
Demonstrate sensitivity to the cultural diversity, gender and sexual orientation of clients being served.
Knowledge of HIPPA and principles of confidentiality.
Demonstrates excellent internal and customer services skills.
Adheres to CCH attendance policy.
Travels between facilities/divisions.
Performs other duties as assigned.

MINIMUM QUALIFICATIONS
Bachelor's degree in nursing (BSN) from an accredited college or university is required. (Must provide official transcripts at time of interview)
Current license as a Registered Professional Nurse in the State of Illinois, required.
