Position Highlights:
Position: Certified Diabetes Care & Education Specialist
Location: Highland Park Hospital
Full Time/Part Time: Full Time (40 hours per week)
Hours: Monday-Friday, 8:00AM-5:00PM
Required Travel: May need to travel between Glenbrook, Highland Park, and Gurnee Tower Court

What you will need:
License:Registered Dietician or Licensed Dietician Required and Certified Diabetes Educator required
Education:Bachelor's degree in Nutrition, Dietetics, or similar area required
Certification:BLS/CPR certification for the Healthcare Provider required and ongoing maintenance of Certified Diabetes Educator (CDE)
Experience:2 to 3 years' previous experience preferred

What you will do:
Education of patients with Type 1, Type 2, and Gestational Diabetes Mellitus
Appropriate assessment and evaluations on patient conditions, follow-up needs, and coordination of health care
Individual and group sessions for Medicare, Newly and previously diagnosed diabetes mellitus, Change in treatment plan
Instructions on insulin pumps, pattern management, and continuous read out sensors
Nutritional prescriptions, sick day management and home glucose monitoring also included for both individuals and in classes
