The Director, Home Health Intake oversees the coordination of agency referrals and assessment of incoming patients to ensure the organization’s capability to meet their health needs. Develops strategic intake processes, utilizes advanced industry knowledge to solve complex challenges, and sets objectives for resource allocation and goal achievement. The Director, Home Health Intake requires an in-depth understanding of how organization capabilities interrelate across the function or segment.

The Director, Home Health Intake :

+ Evaluates and documents comprehensive patient information, identifies payment sources, and determines eligibility to ensure a smooth transition of care.

+ Makes strategic decisions regarding resource allocation, operational tactics, and project initiatives within the department.

+ Extensive collaboration across different departments, and involves leading briefings and department-wide meetings, while maintaining regular interaction with other managers and staff to align departmental objectives.

+ Decisions are typically related to the implementation of new/updated programs or large-scale projects for the function and supporting technical/operational procedures and processes, and implements strategic plans, drives goals and objectives, and improves performance.

+ Provides input into functions strategy.

Required Experience:

+ Bachelors degree or equivalent generally required.

+ 5 or more years of leadership experience preferred.

+ Operations experience; Home Health operations experience preferred.

+ Experience with patient referral/intake process preferred.

+ Homecare Homebase (HCHB) system experience preferred.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$112,400 – $154,900 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
