Job Description
General job duties. Types of procedures required.
Case manage 20-25 patients, seeing about 5-6 per day. Assessment, wound care, IV care, oasis documentation required
Equipment utilized in the unit/dept.
computerized charting with EPIC EMR, give a nursing truck kit with supplies, scale, dressing kits, foley, IV supplies
Required skills and minimum years of experience.
Minimum 1 year home care experience required; oasis experience required
Specific licensures and certifications/registrations required.
BLS required, active RN license
Shift or schedule requirements.
work alone 8-4:30pm Monday-Friday, no weekends, no call
Floating to other units/departments.

Nurse to patient ratio of this unit. (nursing only)
400 total census, each nurse manages 25 patients
Dress Code: scrubs, business casual is okay with white smock

training with RN preceptor for 3-5 days then in the field with assist from staff development team and nursing supervisor

Must be able to document in EPIC without issue
