Schedule: Flexible Part-time; minimum 20 hrs/week, evenings/weekends available
Job Description:
We're seeking a Remote Nurse Educator or Health Coach to provide patient education on chronic disease management, nutrition, wellness, and preventive care via phone, email, or video.
Responsibilities:

Conduct 1:1 virtual health coaching sessions

Develop and send customized care plans

Educate on medication adherence, diet, lifestyle

Record coaching outcomes and update EHR

Create digital wellness materials (as needed)
Requirements:

RN or LPN license in Illinois
-
Background in wellness, nutrition, or public health a plus

Friendly, empathetic communication skills

Tech-savvy and organized
