JOB DESCRIPTION POSITION: INDEPENDENT LIVING SPECIALIST (Blind Service Program) / STATUS: FULL TIME (minimally 30 hr./weekly)
RESPONSIBLE TO: Program Director DESCRIPTION: General scope of duties is to provide effective and efficient IL services; peer counseling, information and referral, advocacy, and skills training to individuals with low vision and the community. Duties include but are not limited to the following:
ESSENTIAL FUNCTIONS: Provide IL Core services: including adjustment counseling, information & referral, advocacy and skills training for consumers with visual impairments as needed to maintain community living status. Skills training may include mobility, writing, communication, cooking etc.: with/without IL aids. Assess consumer to determine magnification needs for the home, make referrals to professionals, select magnification aids and low vision devices. Assist with activities which increase social integration of consumer with visual impairments including the coordination of Blind support group. Conduct and document community education and program marketing. Assist consumers in selection of amplified phones and submit ITAC paperwork to Program Director in a timely manner. Complete purchase requests, maintain inventory and distribution of supplies and equipment for the Elderly Blind Program. Serve IL consumers when assigned. Establish and maintain Consumer Service Records. Develop and maintain computer tracking systems and forms necessary for program data collection and evaluation. Maintain complete confidentiality of any and all information that concerns consumers, potential consumers, and SICIL staff. FAILURE TO DO SO WILL CONSTITUTE GROUNDS FOR IMMEDIATE TERMINATION OF EMPLOYMENT. QUALIFICATIONS: A High School Degree or GED is preferred. Disability Awareness of visual impairments and related experience required. The effective date of revision of this Job Description is May 16, 1997. All employment at SICIL is contingent on the availability of necessary funding.
