What You Will Do
The Medical Social Consultant, as a vital member of the We Care interdisciplinary team, provides professional social work services within a university-sponsored, nurse-managed clinic. This clinic integrates primary care services with public health outreach to serve a diverse and underserved population in the Metro-East St. Louis area.
- Assess patients and their families using the biopsychosocial and spiritual model of care, employing the strengths perspective to determine client eligibility for services.
- Empower patients and their families through counseling on issues such as grief, adjustment to illness, and other psychosocial concerns.
- Educate patients and families about available community and government services (e.g., housing, transportation, financial resources) and facilitate referrals as needed.
- Advocate for patients and families to ensure access to necessary community, governmental, or other support services.
- Supervise MSW students in field practicum placements from the SIUE Department of Social Work, providing mentorship and guidance.
- Develop and coordinate comprehensive service plans for clients, collaborating with other service delivery agencies as necessary. Service plans should outline goals and target dates for achievement.
- Maintain accurate case records and prepare reports required by the Departments of Nursing and Social Work at SIUE, as well as the Health Resources and Services Administration (HRSA).
- Conduct individual clinical interventions, including ongoing support for individuals, families, and potentially small groups.
- Work closely with medical and nursing personnel to integrate social service and medical/healthcare needs effectively.
- Collaborate on research studies related to patient needs and participate in formal evaluations of service delivery at least annually.
Minimum Acceptable Qualifications
Minimum Qualifications:
- Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
