Southern Illinois Center for Independent Living 2135 West Ramada Lane Carbondale, IL 62901 (618) 457-3318 (Voice/TTY) JOB DESCRIPTION POSITION: STATUS: RESPONSIBLE TO: DESCRIPTION: INDEPENDENT LIVING SPECIALIST (ILS)/Transition Coordinator COMMUNITY REINTEGRATION PROGRAM FULL TIME PROGRAM DIRECTOR General scope of duties is to provide effective and efficient IL services to individuals who wish to move from a nursing facility to a community living site. Duties include but are not limited to the following: ESSENTIAL FUNCTIONS: Relocate persons with disabilities residing in nursing facilities into a community residence. Peer Counseling, Skills Training, Advocacy and Information and Referral to Consumers moving from nursing facilities into the community. Coordinate with other agencies/service providers, nursing facilities and home health care agencies, who could assist consumers in goal progress/attainment. Assess individual needs to identify a home and support system in the home which meets safety and accessibility requirements. Make one time purchases, including the completion of vendor bids for equipment. Minor furniture assembly and assistance with moves Establishment and maintenance of case files, which, at a minimum, contain: intake and assessment forms, progress notes, individualized ILSPs, and relevant correspondence, purchase research and information release forms. Timely completion of all SICIL and State required documentation. Submit report to Executive Director for State Quarterly reporting Function within the scope of NBCC Code of Ethics. Advise the Executive Director of circumstances that might warrant attention/services beyond the scope of SICIL purpose(s) and/or operational policies & procedures. REMARKS: Maintain complete confidentiality of any and all information that concerns consumers, potential consumers, and SICIL staff. FAILURE TO DO SO WILL CONSTITUTE GROUNDS FOR IMMEDIATE TERMINATION OF EMPLOYMENT. A Bachelor’s Degree in Rehabilitation, Social Service, or a related field is preferred. Substantial consideration will be given to persons who have a sound knowledge of independent living concepts and experience in the delivery of services to persons with disabilities in ILCs or similar work settings. Travel required.
Significant importance is given to a working knowledge of disability related
issues/concerns, disabilities, and service delivery methods.
