What You Will Do
The Nurse Patient Advocate (NPA) is responsible for guiding patient-centered, quality care in the We Care Clinic and We Care Mobile Health Unit to improve the health of an under-served community. The primary role is to maximize the impact of the Inter-professional Care Team (IPCT).
- Advocate for patients and serve as a liaison to facilitate communication between patients and providers.
- Provide patient education and ensure appointments inside and outside the clinic are scheduled and kept.
- Be available to interprofessional team members for consultation and coordinate urgent or emergent services.
- Review medications, gather personal and family medical history, identify the reason for visits, and complete holistic assessments.
- Interact daily with the interprofessional care team and patients to support patient-centered, quality care.
- Help establish and sustain an interprofessional, collaborative culture within the healthcare setting.
- Perform other duties as assigned by the Director or Clinic Coordinator.

Minimum Acceptable Qualifications
- Licensed as a professional registered nurse in the State of Illinois.
Knowledge, Skills, Abilities - Excellent verbal and written communication skills.
- Organized and able to multi-task.
- Case management experience preferred.
- Health coaching and chronic disease management experience preferred.
- Proficient in Spanish helpful.
