Elevate Care is seeking a detail-oriented Intake Coordinator / Admissions Coordinator to join our team. In this key role, you will manage the admissions process, ensuring a smooth and efficient experience for prospective residents and their families. Your Responsibilities:
Manage Admissions Process – Guide new residents and families through the intake process from start to finish.
Ensure Compliance – Ensure all required documentation and information are completed for admissions.
Communicate with Families & Physicians – Coordinate with physicians, insurance companies, and families to arrange admissions and transfers.
Handle Documentation – Maintain accurate, up-to-date patient records and admissions files.
Provide Information & Assistance – Answer questions about services, financial options, and care plans.
Coordinate with Other Departments – Collaborate with nursing, social services, and other departments to ensure seamless transitions.
Qualifications:
Previous Experience in Admissions or Intake Coordination (Preferred, but not required).
Strong Organizational & Time Management Skills.
Excellent Communication & Customer Service Skills.
Ability to Multi-Task & Work in a Fast-Paced Environment.
Attention to Detail & Accuracy.
Compassionate & Personable with a passion for helping others.
