Job Description
- Assist and advocate for clients to apply for and enroll in government programs (Medicare, Medicaid, SNAP, SSI, MSPs) and health insurance plans including Medicare Advantage.
- Connect clients to community resources for housing, financial literacy, and legal/estate planning needs; refer to licensed professionals when appropriate.
- Work with government agencies and community organizations to stay current on available benefits, policies, and regulations.
- Manage a team supporting MAPD clients across all health and insurance needs, including chronic disease monitoring, hospitalizations, and prescriptions.
- Track client cases and follow through to ensure under-privileged individuals and families receive the support they need.
- Train and mentor team members on social service delivery and compliance with program guidelines.
- Prepare program evaluations documenting outcomes and areas for improvement.
- Supervise Social Workers, all other.
