Key Responsibilities

  • Perform initial and ongoing nursing assessments for senior clients in their homes.
  • Develop, implement, and update individualized care plans in collaboration with physicians, clients, and families.
  • Administer medications, wound care, and other skilled nursing treatments.
  • Educate clients and family caregivers on disease management, health maintenance, and medication compliance.
  • Communicate regularly with our interdisciplinary care team to ensure seamless, high-quality care.

Qualifications

  • Active, unencumbered Registered Nurse (RN) license in the state of Illinois.
  • Valid driver’s license, reliable transportation, and active auto insurance.
  • Previous home health or geriatric care experience is a plus, but passionate new or transitioning RNs are welcome!
  • Strong clinical judgment, communication skills, and a heartfelt dedication to serving seniors.
