Key Responsibilities
- Perform initial and ongoing nursing assessments for senior clients in their homes.
- Develop, implement, and update individualized care plans in collaboration with physicians, clients, and families.
- Administer medications, wound care, and other skilled nursing treatments.
- Educate clients and family caregivers on disease management, health maintenance, and medication compliance.
- Communicate regularly with our interdisciplinary care team to ensure seamless, high-quality care.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the state of Illinois.
- Valid driver’s license, reliable transportation, and active auto insurance.
- Previous home health or geriatric care experience is a plus, but passionate new or transitioning RNs are welcome!
- Strong clinical judgment, communication skills, and a heartfelt dedication to serving seniors.
