GENERAL DESCRIPTION OF RESPONSIBILITIES
The Staff Nurse, under the direction of the Director of Nursing, is responsible for promoting the delivery of coordinated, comprehensive care to patients through the activities of assessment, planning and provision of direct nursing care within the policies and standards of the company.

DUTIES
•Be responsible for the observation, assessment, nursing diagnosis, counsel, care and health teaching of the ill, injured or infirm, and the maintenance of health and prevention of illness of others.
•Assumes responsibility for an assigned caseload; makes planned home visits for
assessment, instruction, supervision, and provision of nursing treatments according to the plan of care using a high degree of ethical, professional and confidential care.
•Develops, implements and evaluates a comprehensive, goal focused, care plan in partnership with the patient, representative (if applicable) and caregivers, and makes updates in accordance with written orders from the physician.
•Collaborates with the patient, representative (if applicable), caregivers in establishing realistic patient/nursing goals and expected outcomes.
•Initiates and maintains continuous contact with the patient’s physician to report the patient’s response to home care, any changes in status, or possible need for additional services.
•Coordinates with therapists, in-home service workers, and others who provide services to each patient to form an interdisciplinary team to ensure ongoing assessment of the patient and consistent and appropriate care.
•Plans for the patient’s discharge from service.
•Administer medications and treatments as prescribed by the patient’s physician.
•Shares the responsibility of on call duty for patients in emergency situations, including home
visits as required.
•Participates in in-service programs, staff meetings, and case conferences
•Recognizes ethical and legal responsibilities involved in patient care, respects patient rights,
and property.
•Accepts changes/additions to schedule during staffing difficulties when necessary.

CRITICAL DEMANDS
•Must be able to meet the physical demands of providing nursing care, ambulating patients,
transferring patients, assisting with exercises, etc.
•Must be able to transfer patients as needed according to proper techniques of body
mechanics to promote patient and staff safety and well-being.

QUALIFICATIONS
•Current registration as a nurse in Illinois.
•Minimum of two years of nursing experience in an acute care or home care setting is
preferred.
•Must be empathetic, nonjudgmental, tactful, responsible, adaptable, organized, and committed to the purpose and objectives of the organization.
•Must have excellent communications skills. Multi-linguial in Spanish and/or Gujarati a plus.
•Must understand the team approach to patient care and be a supportive team member.
•Comfortable working independently in a home-visit setting.
•Basic computer and software skills (use of Internet, smart/cell phone, e-mail etc.).
